I am very new to this and would appreciate any help. I need a form that can handle reference and image upload to be included on our website. I have just downloaded root.zip from here.
My question is that all I am trying to accomplish are - To get 1. Employer Name, 2. Employee Name, 3. Employee Home Address, 4. Employee Hire Date, 5. Is Employee eligible for Rehire Yes or NO, 6. Any discernible mark on employee, 7. Comment...
Each of this will have a box to input the data, and then the person filling the form will upload the picture of the employee. Btowse to the location and attach the image and submit. image should be jpeg and not be more than 40kb
So the data field and the image will be sent to an email address once the visitor clicks the submit button and we can harvest the data from the email address. The person must then be redirected back to the home page.
Is everything I just explain above doable with the form I downloaded? If so, what do I need to tweak to make it work? Please understand that I am a true novice.
